This is something speculative but did you exit the session and start a new savegame OR did it happen when you started a new game while you did not...

I just checked: Fire Rain should only start after 72 hours - if it starts right when you are in the starting area, then there is a bug somewhere.
Did you start a new game with the most recent version?

First: are you sure you actually have the quest completed? When you open the PDA, does the Quest show the 4/4 state? (For example?)
