Known issue with placement of a starter block while connected to a container and then deconnecting (or deconnecting in between)
survival, dediserver, random generated scenario I imagine this may be known issue too, but didn't see it on the list. Placing an SV starter (I think this at least used to happen with other vehicle starters too), leaves a permanent "afterimage" of the starter. This also happens if you activate the starter on hotbar, and then move to another block like in the picture. Changing back to starter block, resumes control of the image, so I think the problem is that game doesn't know how to remove that image once it no longer should be on the map. The afterimage persists if you move out of the area, and return later. When you log out of game and back in, it's gone - but I've seen these images floating in the air far away from original spot when I fly around the planet, so I'm not sure what exactly happens. I think removing client cache may have removed those floating images, but it didn't stop this issue from happening.
its an issue that happens, most of the time, if you pick up the Starter block and place into the ACTIVE slot in your VT Toolbar.
This happens with all vehicle starter blocks, not just the SV. The virtual toolbar seems to be the cause of any such mishaps, because placing them in the normal toolbar removes the phantom used for placing the item, the VT does not for whatever reason.
last time it happened to me was a few days ago while building a base a CV BP got ready to spawn. I had the VT active while spawning..yes, I got a green cubed image of the CV while spawning the CV once. had to quit game, restart PC and log in again to get rid of it.

IT happens when you are connected via Virtual Toolbar, also when you place a Starter Block in the VT to start building something
Heyas, So I was trying to do exactly what the subject says, and every time I would try to do so with the starter block in the connected container's toolbar, I would get a glitch of the green-glowing starter blocks hovering in midair at the spot they occupied the moment I left-clicked. Since the actual blocks then fall to the ground, it looks like the blocks duplicated. However, in each instance they green glowing phantom blocks are intangible. Tossed in screenshot showing what it looks like. Interestingly enough, I'm seeing these phantom blocks in other chunks, or whatever it is you call the world spaces. I'll be flying along and see these green phantom blocks at various altitudes and locations over the terrain well-distant from where I built the SV. Sorry, guys! I did a search before I posted this, but I didn't use the same terminology. My bad... Oh, and as I mentioned- sometimes you see more artifacts of the event well distant of the point of spawn. Attaching a pic of the midair shizzle below...

Reproducibility: Always Severity: Minor, but very annoying if doesn't disappear at all Type: Start Block (Logistic System) Summary: Block-Starter Preview does not disappear if you place a starter while connected via Logistic System Description: If you place down a starter Block while connected to the logistic system, the green block-preview (that showes you where you will spawn the block) doesn't disappear and stays on the playfield. Reloading the save & restarting the server didn't fix this issue always. Sometimes the green preview stays on the playfield, sometimes it moves around when you rejoin the server, but it doesn't disappear. Steps to Reproduce: 1.) Connect to the logistics system. 2.) In the logistics-system toolbar, pick a starter (e.g. SV_StartBlock) 3.) Place down the starter, and you see that the green Preview won't disappear. Did you place the starter whle being connected to the Logistic network and switched or deactivated in between the placement process? If so > known issue
Yes I was connected to the logistic network (as you can see in the screenshot with the yellow hotbar), but no I did not switch between the logistic hotbar and my personal one nor activate/deactivate the logistic connection, within the placement process. I also checked the known issues list before posting this, and I didn't see this topic, so I apologize if it's a duplicate.

I had this happen to me but slightly different I had an extra hv that was not hitboxed outside of the bounding box. The extra ship was not connected to the actual ship thankfully and stayed till I quit to menu and reloaded the save, the bounding box still lit up when my crosshair was on it though. I made a save backup incase I needed it though.
